Instructions:

There is no fee charged for Infant Dedications.

• This form must be submitted at least three(3) weeks prior to your engagement.
• Submission of this form does not imply an approval of your event
• Upon receipt, your form will be placed in the appropriate Ministerial Staff’s box for approval
• Infant Dedications are scheduled to occur one Saturday in each month, except Mother’s Day, Father’s Day and Christmas.
