Please Note: Room request forms must be completed a minimum of 30 days prior to the event you are planning. If not submitted in a timely matter, we cannot guarantee approval of your event. Submission of this form does not imply an automatic approval of your event. Please do not submit your form to the Church Office prior to obtaining an APPROVAL from your Staff Minister.
